Choice and Control: The Impact of Legal Capacity on Equitable Access

This project focuses on identifying and analyzing legal capacity barriers in federally regulated services. It will involve people with disabilities, families, legal experts, and industry representatives to gather lived experiences and insights. Findings will inform future accessibility standards by highlighting systemic issues, recommending solutions, and sharing best practices.

Claim form

This form shows how you spent Accessibility Standards Canada’s funding on project activities for a specific time period.

When to submit the form

  • If you receive your funding through a contribution, you must submit a claim form before each advance quarterly payment.
  • If you receive your funding thought a grant, you don’t need to submit claim forms.

Late submissions may affect your funding or project status. If you anticipate a delay, contact us as soon as possible at Recherche.NAC-Research.ASC@asc-nac.gc.ca .

How to complete the form

  • Fill in your project and organization information (Note: the field “Recipient name” refers to the organization’s name, not an individual)
  • Indicate in which fiscal year quarter the expenses claimed were spent
  • For each project cost category listed, enter how much of our funding you spent during the reporting period in the “Amount claimed” column
    • If you did not spend anything in a category, enter zero (0)
    • The total amount in your claim form must match the total for the same fiscal year quarter in your Forecast of project expenditures form
  • Have the form signed by an authorized signatory

This study will identify best practices for removing barriers and increasing access for people who are blind or partially sighted. It will also identify how people with other types of disabilities may have needs that conflict with the needs of those who are blind or partially sighted.
